At SOMS, we are committed to protecting your privacy and handling your personal and health information with care, confidentiality and respect.

This statement explains what information we collect, how we use it, how it may be shared, and your rights under the New Zealand Privacy Act 2020 and the Health Information Privacy Code 2020.

  • We collect information that is necessary to provide our services, manage appointments and meet our medical and administrative obligations. This may include your name, contact details and relevant health or medical history.

  • We use your personal and health information for the primary purpose for which it was collected, such as managing your care, responding to enquiries, scheduling appointments and supporting clinical communication.

  • We will only share your information where it is directly related to your care, where you have authorised it, or where it is required or permitted by law. This may include sharing with other health professionals involved in your care, such as specialists, GPs, laboratories, radiology providers, pharmacies or hospitals.

What is HealthOne?

HealthOne is a secure electronic record that allows registered health care professionals directly involved in your healthcare to quickly access information such as your test results, allergies, medications, GP summaries and hospital information. HealthOne adheres to the principles of the Privacy Act 2020 as well as the Rules set out in the Health Information Privacy Code 2020. Access is only possible via an approved highly secure healthcare information network which is regularly audited and tested. Privacy auditing is used to check that only those directly involved in your care are accessing your information.

  • Your information is stored securely and protected from loss, unauthorised access, modification or improper disclosure. We use safeguards to protect your information, including secure systems, access controls and staff training.

  • You have the right to request access to the personal or health information we hold about you and to ask us to correct it if you believe it is inaccurate or misleading. If you would like to make a request, please contact our Privacy Officer using the details below.
